FHIR Server Updated

I have upgraded my test FHIR server to the latest FHIR version. The server is found at http://fhir.healthintersections.com.au My server and the specification will roll in synchronisation for the next few weeks in the lead in to the connectathon.

Note that this is pretty much the stable version for the connectathon, so we can all start working on that now.
